What telehealth is
Telehealth means receiving care electronically, by video, phone, secure message or an online questionnaire, instead of sitting in a room with a clinician. This page explains what you are agreeing to when you use it.
Your provider reviews your history, your answers and anything you send, and makes a clinical decision from that. Some things can be handled this way and some cannot.
Who provides your care
Clinical care is provided by licensed providers, and any prescription is dispensed by a licensed pharmacy. The names of the clinical group and the dispensing pharmacy that serve this clinic: [clinic to confirm]
You can ask at any time who your provider is, where they are licensed, and which pharmacy is filling your prescription.
No guarantee of treatment
Consulting a provider is not the same as being prescribed something. Your provider may decide a treatment is not appropriate for you, may ask for more information, may order laboratory work, or may ask you to be seen in person first.
If a provider decides a treatment is not clinically appropriate, you will not be charged for it.
Risks and limits
- A video or connection can fail, and information can be delayed or lost in transit.
- Without a physical examination a provider may miss something that an in-person visit would have found.
- Your provider may conclude at any point that you need to be seen in person, or need emergency care.
- Despite our safeguards, no electronic transmission can be guaranteed secure.
Your responsibilities
The care you get is only as good as what you tell us. You agree to give accurate and complete information about your health, your medicines, your allergies and your history, to tell us promptly if something changes, and to follow the instructions you are given.
You also agree not to share your medication with anyone else.
Emergencies
Telehealth is not for emergencies. If you are having chest pain, difficulty breathing, severe bleeding, a suspected stroke, a severe allergic reaction, or thoughts of harming yourself, call 911 or go to your nearest emergency department.
Privacy
Information you share in a telehealth visit is protected the same way as information shared in the clinic. How it may be used and shared, and the rights you have over it, are set out in our Notice of Privacy Practices.
Visits are not recorded unless you are told and agree beforehand.

Withdrawing your consent
You may withdraw this consent at any time, in writing, without affecting your right to future care. Withdrawing it does not undo care already given, and does not remove records we are required to keep.
